Determinants of procedural outcome of chronically implanted pacemaker and defibrillator leads using the Excimer laser sheath.

چکیده
The extraction of chronic pacemaker and internal cardioverter defibrillator leads is performed for a number of reasons including chronic infection, lead dysfunction, and venous obstruction. Traditionally leads were removed by traction which is associated with a high incidence of failure and serious complications. This has led to the introduction of laser lead extraction (LLE) which is a safe and reliable method with a success rate of over 90%. We prospectively examined the procedural success and safety of LLE over a two year period in our institution. We also examined whether procedural outcome could be predicted by baseline variables.

Initial experience in the extraction of chronically implanted pacemaker leads using the Excimer laser sheath.
OBJECTIVE To assess the safety and efficiency of the Excimer laser sheath in extracting chronically implanted pacemaker leads. PATIENTS Eight patients were studied (one female, mean age 62 years, range 34 to 77) with 17 pacemaker leads (five atrial, 10 ventricular, two implantable defibrillator). The mean implantation time was 65 months (range 23 to 188). The indications for lead extraction w...

OBJECTIVES The purpose of this study was to evaluate the safety and effectiveness of pacemaker lead extraction with the excimer sheath in comparison to nonlaser lead extraction. BACKGROUND Fibrotic attachments that develop between chronically implanted pacemaker leads and to the venous, valvular and cardiac structures are the major obstacles to safe and consistent lead extraction. Locking sty...
متن کامل[Initial experience with excimer laser-assisted pacemaker and defibrillator lead extraction].
Explantation of chronically implanted pacemaker or defibrillator leads can be technically demanding. We present the results of the first 44 leads extracted with excimer laser sheaths in 25 patients with a median lead implantation time of 7 years (range: 2 to 20 years). Indications for extraction were lead fracture in 17 patients, pocket infection in 4, septicemia in 2, and lead endocarditis in ...
متن کاملLaser assisted extraction of pacemaker and implantable defibrillator leads.
OBJECTIVE To analyze the results of laser-assisted extraction of permanent pacemaker and defibrillator leads. METHODS We operated upon 36 patients, whose mean age was 54.2 years, and extracted 56 leads. The reasons for extracting the leads were as follows: infection in 19 patients, elective replacement in 13, and other causes in 4 patients. The mean time of catheter placement was 7.5+/-5.5 ye...
متن کاملTransvenous extraction of a 26-year-old Accufix atrial lead using TightRail rotating dilator sheath.
With the increasing number of implanted pacemakers and implantable cardioverter defibrillators, removal is required more frequently. Presently described is the transvenous extraction of a 26-year-old Accufix atrial lead using a mechanical dilator sheath. A 50-year-old male patient was admitted to the clinic with a pacemaker pocket infection. The atrial lead was an Accufix Bipolar J-Atrial activ...
